Chosen Name and Gender Identity Profile Change FAQ

General

Yes, please note that our student information system is not able to display both your official name and your chosen name. The update to your first chosen name will be made across the systems as your official first name. Both your academic and financial record will reflect your chosen name.

If you have any questions regarding your chosen name and gender identity profile, please contact yorknow@yorku.ca.

Fill out an Alumni name change form and email it with the required documents to yorknow@yorku.ca.

In higher education, only the first name could be a chosen/preferred name, and for that no legal paperwork is needed. For middle and last name changes, we need two legal government ID’s (i.e. driver’s licence, birth certificate, Canadian or international passport, Canadian citizenship card, certificate of Indian status, change of name certificate or permanent resident card) to make the name change.

As a staff member - reach out to your supervisor/manager to make the name change with Human Resources - hrhelp@yorku.ca. Also, you can make your own name change as a staff member in your HR Self Serve online.

As a student – if you’d like to use your chosen/preferred name, please fill out the Chosen Name and Gender Identity Profile Change form.
 

Email/eClass

No, this can be updated by the student directly in eClass at any time.

As an undergraduate student – email askit@yorku.ca to have them update your first name so that your @my.yorku.ca email account will display your preferred/chosen name. You may also respond to the Records Unit once your chosen/preferred name has been updated to proceed with this on your behalf.
As a graduate student – the first name displayed in Outlook is updated daily based on what is in the student system.

Official Documents

It is your choice whether your diploma has your chosen/preferred name or your official/legal name on it. If you’d like your official/legal name on your diploma, please fill out a Student Name Change Request Form. Otherwise, your chosen/preferred name will be reflected on the diploma.
